How far do you go to look after your health?

I am writing a book chapter about being healthy and I am going to do a series of Discussions of the Day to see how far you take it.
We know that ideally we should eat a healthy diet and exercise. But do we manage to control this? We are often rushing around juggling our work:life balance and looking after ourselves goes out the window.
We may not find time to plan meals and cook healthy food. Most of us love a glass of wine or a skinny latte. And sugary foods can make us feel better in the short term. We often love the things we should not have!!
How far do you go to look after your health?
Do you eat a healthy diet?
Do you eat your five a day of fruit and vegetables?
Do you avoid from processed food?
Do you avoid sweets and crisps?
Do you avoid tea, coffee, sugar drinks and alcohol?
Do you avoid cigarettes?
Do you exercise daily?
Do you get 8 hours of sleep a night?
Do you look after your wellbeing?
